Fair Political Practices Statements

Campaign Finance Statements (Forms 460, 470, 496 and 497) and Statements of Economic Interests (Form 700) must be filed electronically using NetFile.

Campaign Contributors

If you intend to make any of the following contributions and you do not have a NetFile account, you must set up an account prior to filing.

  • Major Donation (Form 461)
  • Independent Expenditure (Form 496)
  • 24-hour Contribution (Form 497)

Government Code Section 87200 Filers

City Councilmembers are elected officers identified in Government Code Section 87200:

  • Councilmember Richard Mehlinger

Copies of the Form 700 Statements of Economic Interests filed by the above elected officers may be obtained by visiting the City of Sunnyvale Office of the City Clerk at 456 W. Olive Av., 94086 or the FPPC at 1102 Q Street, Suite 3050, Sacramento, 95811. Copies of the Statements of Economic Interests for the City's elected officers are also available in electronic format in NetFile. Statements of Economic Interests for certain state and local government agency elected and appointed officers are also available on the FPPC website.

Payment to Agency Report (Form 801)

FPPC Form 801 is used to report certain payments received by state and local government agencies.

Agency Report of Public Official Appointments (Form 806)

FPPC Form 806 is used to report additional compensation that officials receive when appointing themselves to positions on committees, boards or commissions of a public agency, special district, and joint powers agency or authority.
